This Notice of Privacy Practices describes how UTI Network may use and disclose your Protected Health Information (PHI) and your rights regarding your health information.
Our Role
UTI Network coordinates logistics, physician access, and technology to route specimens to certified laboratory partners. We are not a healthcare provider or laboratory. Testing and medical services are provided by independent, licensed clinicians and CLIA-certified laboratories.
How We Use and Disclose PHI
We may use and disclose PHI for:
- Treatment: Coordinating specimen collection, transport, and results delivery between patients, physicians, and laboratories.
- Healthcare Operations: Managing logistics, quality assurance, and technology services for our laboratory partners.
- As Required by Law: When required by federal, state, or local law.
Your Rights
You have the right to:
- Request restrictions on how your PHI is used or disclosed
- Request confidential communications
- Access and obtain a copy of your health information
- Request amendments to your health information
- Receive an accounting of disclosures
- Receive a paper copy of this notice
HIPAA Compliance
All PHI handling follows HIPAA-aware processes. We execute Business Associate Agreements (BAAs) with our laboratory partners as appropriate and maintain proper safeguards for all protected information.
Complaints and Questions
If you believe your privacy rights have been violated or have questions about this notice, please call 1‑800‑UTI‑FREE (1‑800‑884‑3733). You may also file a complaint with the U.S. Department of Health and Human Services.
You will not be retaliated against for filing a complaint.
